WHAT IS PERSONALIZED HEALTHCARE?
Personalized healthcare represents a transformative shift in how patients receive care and interact with healthcare providers across their entire healthcare journey. At its heart, personalized healthcare is about:
- Shifting from a ‘one-size-fits-all’ to a ‘my-size-fits-me’ approach to wellness, prevention and treatment.
- Using health data and digital tools to inform decisions that deliver the right intervention to the right person in the right setting at the right time.
- Creating a health system that achieves better outcomes, makes better use of resources and enhances your experience.

Realizing the potential of personalized healthcare will require our health system to change in many ways. At a high level, this transformation will depend on:

Our mission is to enhance the impact of complementary efforts in personalized healthcare across Canada by sharing knowledge, driving cross-sector collaboration, and fostering dialogue with patients and the public.
PHCC is built around three core objectives:
Engage the Public
Define priorities for a more transparent, equitable and inclusive healthcare system by engaging patients and the public in the conversation about the future of personalized healthcare.
Grow Awareness
Grow awareness and share knowledge of personalized healthcare initiatives across Canada by maintaining a digital catalogue of resources and ongoing initiatives.
Foster Partnerships
Foster cross-sector, multidisciplinary partnerships across Canada by providing a platform and opportunity for dialogue, engagement and collaboration around personalized healthcare.
